What is a Collaborative Care?

Collaborative Care Model (CoCM) is a program that integrates behavioral health services into your child’s primary care, focusing on treating conditions like anxiety and depression within the pediatric office. Our dedicated Behavioral Health Care Manager (BHCM), will work closely with your family to provide guidance and coordinate treatment with your doctor and a psychiatric Nurse Practitioner as needed. The care team checks in regularly to monitor your child’s progress.

Who is appropriate for Collaborative Care?

Collaborative care comprises a focused approach that currently targets depression and anxiety in patients 10 years and older. Your Primary Care Physician (PHP) and the BHCM will determine if the patient is a good fit for CoCM or if you may benefit from working with our social work care manager.

Treatment Overview

Create a treatment plan tailored to your child’s needs, focusing on skill-building and emotional understanding.
Set realistic and relevant goals for our time together
The length of treatment can range from 3 months to 12 months. The care plan is reviewed every 4-5 visits.
Use your child’s strengths to empower them and give them the tools for success.

How to Get Involved

Your PCP may refer you during an appointment or over the portal if they believe you will be a good fit. If you think your child may be a good fit, please reach out to your provider directly. They will work with the BHCM to determine what care pathway would benefit the patient best.

Who is involved?

Behavioral Health Care Manager: Tatum Sahli, MHC-LP

She works with our three providers on-site and collaborates closely with your child’s doctor to screen for and address behavioral health concerns, providing guidance and consulting with a psychiatric consultant as needed. Depending on the circumstances, she may also be able to collaborate with the school or other providers.

Pediatrician: Dr Cordaro, Dr. Tfankji, Dr. Barr

Your primary care provider (PCP) oversees all aspects of your care, working closely with the care team to ensure you receive the best treatment. They diagnose conditions, manage prescriptions, monitor your progress with the care manager, and may consult the team psychiatrist if needed. Your pediatrician will refer you to the program based on their clinical observations.

Psychiatric Consultant: Holly Brown

The psychiatric consultant advises the PCP and BHCM on diagnoses and treatment options, especially if initial treatments aren’t effective. They regularly review patient progress with the BHCM.

Social Work Care Manager: Robin Schepp

Ideal if your child has moderate to complex medical, behavioral, or developmental needs that require coordinated care across different settings, such as hospitals, schools, and community services. She may also assist in finding the right care for your child if we believe that the CoCM program is not the best fit for them.
